T.E.A.C.H. Early Childhood® Minnesota Pre-Authorization Request Form

T.E.A.C.H. Minnesota uses the information on the Pre-Authorization form to set up payment for your classes with the college(s) you are attending. The T.E.A.C.H. counselors will email you a copy of the payment letter once it is sent to your college(s). Complete and submit a new form for each term by the following deadlines:

Pre-Authorization Form SEMESTER Deadlines:

Fall Semester	Spring Semester	Summer Semester
August 1^st	December 1^st	May 1^st

Pre-Authorization Form QUARTER Deadlines (Rasmussen University):

Fall Quarter	Winter Quarter	Spring Quarter	Summer Quarter
September 1^st	December 1^st	March 1^st	June 1^st

Below, fill the course information for each class you would like T.E.A.C.H. scholarship to pay for this term. Be sure to include your student ID number for each college where you are taking classes. If your classes change, please complete a new Pre-Authorization form and submit it.
